 Models for Behavioral Health Integration - The IAFP Clinically Integrated Networks committee and the Illinois Psychiatric Society have collaborated to compile an array of successful models throughout the state.  Each practice included in this list has a contact person attached, so you may obtain additional information or ask questions.  

Great Lakes Practice Transformation Network now enrolling! Northwestern University has opened enrollment for the Great Lakes Practice Transformation Network (GLPTN), one of 29 Practice Transformation Networks nationwide.The GLPTN will support organizations at no cost through health care quality compliance, transitions to value-based care, and engaging patients to be responsible health care consumers. Primary care physicians are eligible to participate. Your organization is not eligible if you currently participate in any of the following: Medicare Shared Savings Program (MSSP), Pioneer Accountable Care Organizations (ACOs), the CMS Multi-Payer Advanced Primary Care Practice , or are receiving services from one of the other 28 CMS-designated PTNs. Providers who do not accept any Medicare or Medicaid patients are also ineligible. Learn more here:

H3: Healthy Hearts in the Heartland:  Resources for all and a quality improvement program for small practices (<10 providers) in Northeast Illinois to improve cardiovascular outcomes.  Must have an E.H.R. system.

NCQA Toolkit:  Achieving NCQA PCMH Recognition: A toolkit for practices seeking to apply. The toolkit is publicly available. The Illinois practices that contributed to the development of this toolkit include Family Care Associates in Effingham, Prime Care in Chicago (5 sites), and University of Illinois Pediatrics in Peoria. The toolkit would not have been possible without their willingness to participate and share.
