Alvia Siddiqi, MD, FAAFP

Dr. Alvia Siddiqi joined Advocate Physician Partners (APP) in July 2015 as Medical Director of the Advocate Accountable Care Entity (ACE) with nearly 100,000 covered Medicaid lives. Currently, she serves as Medical Director with APP. In addition to her oversight of the Advocate Medicaid ACE transition, she is a member of the APP QI, UM, Peer Review & Grievance, and CI Design committees. She also serves as Medical Advisor to APP PCMH transformation initiatives and supports APP Care Management and Medicare Patient and Family Advisory Council (PFAC).

Prior to joining Advocate, Siddiqi served as Medical Director of the Illinois Health Connect Primary Care Case Management (IHC PCCM) Medicaid Program. IHC PCCM currently includes nearly 5600 medical homes as part of their Primary Care medical home network responsible for the care of nearly 1.6 Million Medicaid recipients. 

Dr. Siddiqi is a Fellow of the American Academy of Family Physicians (AAFP) and continues her clinical practice caring for underserved patients in the Northwest suburbs at Vista clinic, affiliated with Cook County Health & Hospital Systems. She is an appointed member of the national AAFP Commission on Quality and Practice and represented AAFP on the National Quality Forum (NQF) Measures Application Partnership Medicaid Task Force. She has served as a board member of the Illinois Academy of Family Physicians since 2009. She has also been active with the IAFP public health committee and public relations task force.

She serves on numerous local and statewide committees including the HFS Medicaid Advisory Care Coordination Subcommittee, Chicago Area Patient Outcomes Research Network (CAPriCORN) Patient and Clinician Advisory Committee, Illinois State Medical Society (ISMS) Council on Economics, Illinois Rural Health Association (IRHA) Mental Health Committee, and Illinois Workforce Investment Board (IWIB) Healthcare Task Force. Dr. Siddiqi also co-leads the state’s Children’s Health Insurance Program Reauthorization Act (CHIPRA) Quality demonstration project Care Transitions workgroup.

Her graduate medical education was completed at Loyola University Chicago, Stritch School of Medicine in 2005. During her final year of her Family Medicine Residency Program at Macneal Hospital in Berwyn, Illinois, she served as Chief Resident while co-facilitating an Introduction to the Practice of Medicine (IPM) course for first year medical students at Loyola University Chicago, Stritch School of Medicine. Most recently in March, 2014, she completed a Medical Home training Program offered in conjunction with the Sixth National Medical Home Summit in Philadelphia, PA.
